Understanding Late Onset Hypogonadism (Andropause)

Below normal or low-normal blood testosterone levels accompanied by symptoms of testosterone deficiency is classified as late-onset hypogonadism..

It is often referred to as male menopause or andropause.

About the Condition

Late-onset hypogonadism in men causes dramatic changes in moods, energy levels, feeling of well-being, sleep patterns, muscle strength, muscle mass, fat distribution, sexual desire and sexual performance. Scientific studies show that low testosterone affects up to 37% of all men over the age of 40 and yet less than 1% of men are treated.

  • Below normal or low-normal blood testosterone levels accompanied by symptoms of testosterone deficiency.
  • Decline in testicular function independent of patient age, but general in age > 30 years.
  • Less than 7/100 treated in USA, 1.5/100 Australia and 1.1/100 UK.

Late-onset hypogonadism (low testosterone) is the biggest single factor affected men's health today. There is a significantly higher risk of diabetes, metabolic syndrome, obesity, depression, heart disease and prostate disease in men with low testosterone. Symptoms of Late Onset Hypogonadism (Andropause)

  • Decreased libido and erectile quality
  • Lowered energy
  • Fatigue (depression, anger, anxiety)
  • Mood changes
  • Slow beard growth, fine facial wrinkles
  • Decreased body hair (feminization)
  • Testes atrophic (shrink) if condition long-standing
  • Normal voice pitch
  • Decreased bone mineral density and possible resulting osteoporosis
  • Normal skeletal proportions, penis length, scrotal rugae, prostate size
  • Decreased lean body mass and muscle strength
  • Increased abdominal fat
  • Rudimentary breast development (man boobs)
  • Blood testing is a biochemical evaluation requests levels of testosterone, free testosterone, SHBG and Free Androgen Index. Ignore laboratory lower end of 'normal' reference range because symptomatic male with testosterone reading up to 50% > lowest end of range may be testosterone depleted.

Causes of Late Onset Hypogonadism (Andropause)

The late-onset hypogonadal male is the largest under-diagnosed group of all testosterone deficient individuals. This group consists of middle aged and older men who exhibit symptoms associated with lowered testosterone levels, but do not have primary or secondary hypogonadism.

The late-onset hypogonadal male’s symptoms are often non-specific and include lethargy, sleep disturbances, loss of libido, irritability, anxiety, reduced concentration and depressed mood. This individual will usually have a testosterone blood test that is at the bottom end of the “normal” range which may often result in a medical practitioner prescribing anti-depressants to treat symptoms rather than the underlying cause.

Diagnosing the symptoms which many men experience due to late-onset hypogonadism can be further complicated by pre-existing medical conditions such as obesity, diabetes and chronic illness.

Causes of late-onset hypogonadism include:

  • Stress
  • Aging
  • Alcohol
  • Smoking
  • Recreational drug use
  • Acute critical illnes, burns,major trauma or surgery
  • Chronic disease and its treatment
  • Prescription medications (especially pain killers, glucocorticoids)

Treatment for Late Onset Hypogonadism (Andropause)

Late-onset hypogonadism is treated with the hormone testosterone.

Regardless of the underlying cause of the testosterone deficiency the treatment is universally testosterone supplementation. Testosterone replacement therapy (TRT) aims to restore circulating testosterone concentration to normal in men with all degrees of hypogonadism. TRT is highly effective in restoration of blood testosterone levels to the normal ranges and safely and effectively resolves most if not all symptoms associated with testosterone deficiency.
