About the Condition

Erectile dysfunction (ED) is a very common problem in men. It generally is a symptom of either a physical or psychological issue or combination of both. ED is a separate issue to low libido (little interest in sex), however restoration of sexual interest can have a positive influence on erectile function.

One in every five men over the age of forty experience some degree of ED. With each decade of age the incidence of ED increases. In the developed world two out of every three men over seventy years of age experience erectile problems.

Symptoms of Erectile Dysfunction

Erectile dysfunction (ED) is the lack of ability to achieve or sustain a satisfactory erection. This often results in frustration, embarrassment and feelings of inadequacy in the man which may in turn create psychological issues which further exacerbate the ED problem. Most commonly ED is the result of physical changes such as prostate surgery or the use of some prescription medications, however there may be a number of contributing factors that when combined result in ED. There is a growing body of evidence that suggests that ED is a valid predictive marker for undiagnosed heart disease.

Most commonly ED is the result of physical changes such as prostate surgery or the use of some prescription medications, however there may be a number of contributing factors that when combined result in ED including performance anxiety, relationship issues and premature ejaculation.

Low testosterone levels result in

  • Low sexual drive
  • Lack of arousal
  • Lack of desire
  • Inability to achieve orgasm
  • Loss of erotic thoughts / fantasy
  • Loss of early morning erections

Low testosterone and ED are most often allied issues and addressing low testosterone levels generally lead to improved erectile function, increased energy levels, improved libido and overall sexual function.

Causes of Erectile Dysfunction

Erectile dysfunction can be the result of single event or multiple factors:

  • Drugs (anti-depressants (SSRI's), blood pressure medication and Nicotine are most common)
  • Hormonal Disorders (pituitary gland tumor; low testosterone)
  • Psychological causes: stress, mental disorders, negative feelings, other psychological problems.
  • Pelvic surgeries can cause damage of the nerves and blood vessels.
  • Lifestyle: alcohol and drugs, obesity, cigarette smoking.
  • Arterial Disorders: Any of a number of conditions that can affect an artery. Arteries are blood vessels that deliver oxygenated blood from the heart to various parts of the body.
  • Neurogenic Disorders : Weaken muscles and cause nerve degeneration.
  • Venous Leak: A result of certain veins not closing well enough to retain blood, and erection is lost.
  • Peyronie's Disease - A problem with soft tissue of the penis, causing abnormal curvature.

Treatment for Erectile Dysfunction

The most common treatment for ED is the use of drugs called PDE5 inhibitors. These tablets are best known by their trade names Viagra, Cialis and Levitra. In 70% of men they work by blocking an enzyme in the penis which is stopping the erectile response to sexual stimulation. They work for varying periods of time.

What PDE5 inhibitors do not do is enhance desire for sexual activity, improve arousal or orgasmic pleasure. This is the role of testosterone.

Alternative options for enhancing erectile activity are: Vacuum devices – a pump placed over the penis which draws blood into the penile tissue via suction Injection treatments (Caverject) which is injected directly into the penis resulting in increased blood flow.

Penile prosthetic implants – surgically implanted elastic cylinders are inserted into the penis as is a fluid filled reservoir filled with saline and a manual pump mechanism in the scrotum. Surgery is usually the last option to manage ED.

It is vital that men with ED also be properly assessed for their testosterone status. An initial self-assessment for testosterone status can be undertaken using the Aging Male Symptoms (AMS) quiz.

The AMS questionnaire is a 17 question self-rating symptom-based questionnaire with three key domains of assessment - mind (5 questions), body (7 questions) and sexual (5 questions). Responses to each question are assigned a rating 1-5 (none to extremely severe) and the total sum of all subscales provides a total score. Scores can range from a total low of 17 to a maximum of 85, with a complaint score measuring greater than 50 considered severe.

The AMS questionnaire is well suited to assist in both the diagnosis of testosterone deficiency and for the monitoring of treatment in patients using testosterone replacement therapy.
